VACANCY
:

Direct Care Staff:  Holyoke, MA - 4 Full-Time Positions - Varied Hours

REPORTS TO:

Program Director or designee


REQUIREMENTS
;

HS Diploma/GED.  Experience working in a residential facility.  Experience working with homeless persons or other disadvantaged populations.  Valid drivers license and reliable transportation required.  Bilingual preferred.

RESPONSIBILITIES:

Provide case management, temporary shelter and all ancillary support services to assist families in gaining self-sufficiency and locating and securing safe permanent housing.  Insure that all persons receive safe, decent care and services while in shelter.  Develop monitor and review assigned family cases in the family self-sufficiency plans.  Insure that intakes are done for all families entering shelter program.  Insure that regular case management and housing search services are in place for all families in shelter.  Make sure that all case notes and client records are kept neat orderly and current at all times.  Establish and maintain solid working relationship with all area service providers.  Insure that shelter sire is maintained properly and meets all requirements of basic standards of housing.  Provide Case Management and Housing Search Services to all assigned cases in shelter.  Insure that families receive needed services and obtain safe permanent housing in a timely fashion.  Submit all required paperwork and on time as directed.  File reports to DTA and supervisor as directed.  Maintain and participate in a scheduled on-call process.  Report all serious incidents to supervisor immediately.  Attend regular staff meetings.  Maintain client files for completeness and accuracy.  Attend all meetings as required.  All other duties as assigned.

SALARY:

$26,000 - $28,000
